Abstract

Introduction. Face-to-face CBT (fCBT) is the established treatment of choice for anxiety disorders in children and adolescents but is often not accessible. Computerized CBT (cCBT) is proposed as an alternative to fCBT, due to its accessibility and promising, previous research. However, respective and specific meta-analyses are scarce, drop-out rates are high, and moderators of cCBT are unclear. This paper investigates (1) differences in cCBT/fCBT treatment outcomes (drop-out and remission rates, at post-treatment and follow-up); (2) moderators of cCBT outcomes; (3) differences in moderators to fCBT. Methods. 7 fCBT (n = 341; mean-age = 10.77; female = 50.59%) and 5 cCBT (n = 157; mean-age = 10.71; female = 58.6%) studies were identified. Effects of treatment conditions and predictor variables on treatment outcomes were analyzed through binomial logistic regression analyses. The AIC compared cCBT and fCBT regarding the combined influence of the predictors on treatment outcomes. Results. Compared to fCBT, cCBT showed worse treatment outcomes at post-treatment (cCBT: 59.84%; fCBT: 39.63%; p < 0.001) and follow-up (cCBT: 48.82%; fCBT: 33.75%; p = 0.003). For cCBT, at follow-up generalized anxiety disorder and separation anxiety disorder with panic showed more promising results than other disorders (odds ratio decreased by 0.103% and 0.189% respectively. Conclusion. cCBT is advised as an alternative or addition to fCBT only if fCBT is not accessible and a sound cCBT rationale is developed. However, a future research approach, centered around complex systems theory, still needs to develop such a treatment rationale.
