Abstract

In mental health care in the Netherlands, patient aggression is prevalent in closed psychiatric care settings. The current study focused on examining the effectiveness of the Crisis Monitor (an instrument consisting of four weekly observation scales and two daily scales) designed to predict and reduce aggression in acute psychiatry. Although the Crisis Monitor showed to be of value in former publications, previous research in CTP Veldzicht (a secure psychiatric hospital) indicated that the Crisis Monitor does not reduce the incidence of aggression in that setting. Therefore, the present study aimed to investigate whether the Crisis Monitor could predict the occurrence of aggression incidents within CTP Veldzicht. Specifically, the study examined the four weekly observation scales of the Crisis Monitor: Kennedy Axis V, de Schaal Voor Gevaar, The Social Dysfunctioning and Aggression Scale (SDAS), and the Brief Psychiatric Rating Scale (BPRS). In this study, a binary logistic regression was used to investigate the association between observation scales and the occurrence of aggression incidents, and negative binomial regression to investigate the association between observation scales and the number of incidents. In both analyses, the SDAS had a predictive function. Additionally, the BPRS demonstrated a predictive function in a multivariate model with the three other observation scales in both analyses. Based on the results it can be concluded that the Crisis Monitor as a whole does not predict aggression incidents within CTP Veldzicht. Although the SDAS has shown predictive value for aggression incidents, concerns about its usefulness may exist. The Crisis Monitor is anticipated to have greater utility within acute psychiatry settings than institutions like CTP Veldzicht.
