Abstract

Female Sexual Dysfunctions (FSD) immensely burden affected women and their partners. Women with gynecological conditions (e.g., endometriosis, gynecological cancer, urogynecological conditions) are at increased risk of developing FSD. Yet, little attention is paid to elucidating risk factors for FSD in this vulnerable group. Research in the general public has highlighted numerous biological, psychological, and sociocultural risk factors for FSD, for example, chronic diseases and mood and anxiety disorders. The present study was aimed at investigating (1) risk factors for FSD in women with gynecological conditions, (2) differences in risk factors among the specific gynecological conditions, and (3) the association of these conditions with sexual distress. Data are available from a nationally representative (based on age and state in Germany) online survey with n = 2,100 female respondents, of which n = 400 have a gynecological condition. Statistical analyses included Chi-Square tests, logistic regression analyses, and linear regression. Results revealed that younger age, avoidant and disorganized attachment style, higher education, anxiety disorder and not knowing one’s sexual orientation were significant risk factors for FSD. Differences emerged between the gynecological conditions. For women with endometriosis, sexual orientation was the only significant risk factor. Risk factors for women with gynecological conditions were education and sexual orientation. For women with urogynecological conditions, anxiety disorder, depression, avoidant and disorganized attachment style, and sexual abuse were significant risk factors. Having gynecological cancer and urogynecological conditions predicted sexual distress. The study emphasizes the need for personalized and condition-specific care in managing FSD in women with gynecological conditions.
