Abstract
Introduction: The Green's Word Memory Test (WMT) is a performance validity test researched for its value in distinguishing sufficient performance from underperformance in the dementia population. Besides the classical pass or fail criteria, the WMT allows for a comprehensive profile analysis to increase specificity. While extensively researched in late-onset dementia patients, its validity in the young-onset dementia (YOD) population remains unclear. This thesis aims to validate the WMT in the YOD population. Method: Archival data from the Alzheimer Center Groningen (ACG) was used to sample the patients. Inclusion criteria included patients categorized as young by the ACG, with a probable diagnosis of dementia and performance of the WMT. Additionally, the influence of the severity of the cognitive impairment was investigated using the Mini-Mental State Examination (MMSE) screening tool. Thirty-two participants were included in the final sample. Results: While the specificity of the WMT was 19% when using the pass or fail criteria on the easy subtests, it was 100% when using the profile analysis. The severity of cognitive impairment, as indicated by the MMSE score, had significant effects for the subtests Delayed Recall, Multiple-Choice, and Paired Associate Recall. Lastly, a cutoff value of 30 for the profile analysis was found to be the most accurate cutoff in the current sample. Discussion: Taking into account the small sample size and potential systematic differences between patients who performed the WMT and those who did not, the current thesis found evidence supporting the inclusion of the WMT in the YOD assessment.
